Requirements for the Renewal of Passports
The applicant must come in person and deposit the following:
- Expired passport booklet.
- Birth Certificate issued from the year 2004 and onwards, with event number and legalized.
- If you DID NOT obtainDominican citizenshipby birth, you must present a Citizenship Transcript issued from the year 2004 and onwards, with event number and legalized.
- Identification document, exclusively the new Dominican national identification card.
- If the applicant is a minor one of the parents must present the Dominican national identification card. As well as, complete the Child Authorization Form.
- Two (2) front photographs, size 2” x 2” and with white background. The ears must be visible and without any jewellery on ears, face or neck. One of the photos must be stamped by the institution where it was taken; indicating the date it was taken as it is only valid for one (1) year.
- Marriage/Divorce Certificate issued from the year 2004 and onwards, with event number and legalized, for name changes. As well as, for persons who carry a married name either on the passport or secondary identity document that will be used in the application (if applicable).
- Passport fee of USD135.00 cash only.
- Passports are issued at the consulate in New York City. Therefore, an additional payment of USD45.00 is required for deliveries Toronto-New York and New York-Toronto, cash only.
- If you wish to have us mail you your new passport please provide us a Canada Post prepaid express envelope.
Requirements for the Renewal of Passports for Damage, Loss or Theft
The applicant must come in person and deposit the following:
- Damaged passport booklet (if applicable).
- Identification document, exclusively the new Dominican national identification card.
- Birth Certificate issued from the year 2004 and onwards, with event number and legalized.
- If you DID NOT obtainDominican citizenshipby birth, you must present a Citizenship Transcript issued from the year 2004 and onwards, with event number and legalized.
- A Report of Loss/Theft from the local police where your passport was lost or stolen.
- A Sworn Declaration issued by the Consulate and the service fee of USD40.00 cash only.
- If the applicant is a minor one of the parents must present the Dominican national identification card. As well as, complete the Child Authorization Form.
- Two (2) front photographs, size 2” x 2” and with white background. The ears must be visible and without any jewellery on ears, face or neck. One of the photos must be stamped by the institution where it was taken; indicating the date it was taken as it is only valid for one (1) year.
