You must submit in person or by mail the following documents:

• Photocopy of expired passport or Birth Certificate.
• Photocopy of Dominican National ID (cédula) or other if applicable.
• State your marital status, city where you plan to stay while in Dominican Republic, colour of your skin, eyes and hair, your height, your weight, if you have any visible marks such as tattoos or significant scars, if so please indicate what they are and in what area of your body they are found, your occupation and your parents’ full names if not submitting copy of Birth Certificate.
• Two passport photos 2″x2″, taken and stamped not more than 12 months preceding your request, no earrings and the ears must be visible.
• Service fee of USD20.00, if in person cash only, if by mail money order payable to the Dominican Republic Consulate.
• You may enclose an express prepaid envelope, if not we may mail it to you by regular mail free of charge.
