You must submit in person or by mail the following documents:
- Visa Application Letter in Spanish to the Consulate stating your name, nationality, occupation, as well as information about the expiry and/or loss of residency permit.
- Completed Visa Application Form ( English | Spanish )
- A photograph, the frame size must be 2” x 2” and stamped indicating date of photograph; photograph must not be older than 6 months. The photograph must show the full front view of the head, with the face in the middle of the photograph, and include the top of the shoulders.
- Original passport or travel document with at least 18 months of validity
- Photocopy of all used pages on the passport or travel document
- Original National Police Check(s) with fingerprints for those who have been out of the Dominican territory for more than a year (if applicable) from the country/countries where you have resided in the past 5 years and it must be legalized and translated at our office for a fee of USD100.00 for the legalization and USD40.00 for the translation, if in person cash only, if by mail money order payable to the Dominican Republic Consulate. Note: If the Police Check is from outside of Canada and the country where it’s from is part of the Apostille Convention the Police Check must be Apostille at the country’s foreign affairs office or if the country is not part of the Apostille Convention but has a Dominican Republic Consulate then it must be legalized at that Consulate or if there is no Dominican Republic Consulate within that country, then you may certify it at the country’s Embassy or Consulate here in Canada then legalize it at our office.
- Certification from the General Directorate of Immigration (GDI), accrediting your immigration status in the Dominican Republic.
For renewal:
- Legible photocopy of the expired Residency Card and Dominican Identity Card.
For loss:
- Original Police Report and must be legalized and translated at our office for a fee of USD100.00 for the legalization and USD40.00 for the translation, if in person cash only, if by mail money order payable to the Dominican Republic Consulate. Note: If the document was lost outside of Canada and the country where it was lost is part of the Apostille Convention the Police Report/Judicial Document must be Apostille at the country’s foreign affairs office or if the country where you lost it is not part of the Apostille Convention but has a Dominican Republic Consulate it must be legalized at that Consulate or if there is no Dominican Republic Consulate within that country, then you may certify it at the country’s Embassy or Consulate here in Canada then legalize it at our office.
Fee:
- Processing fee of USD90.00, if in person cash only, if by mail money order payable to the Dominican Republic Consulate.
NOTE: WHEN MAILING YOUR DOCUMENTS PLEASE INCLUDE A SELF-ADDRESSED PREPAID EXPRESS ENVELOPE.
IMPORTANT: The processing time is 5-15 business days. The visa is valid for two (2) months and one entry.
